Left atrial myxoma associated with coronary artery disease - uncommon challenge

Abstract

Atrial myxomas are the most common primary heart tumors and the early diagnosis is rather difficult, since the clinical aspects are often non-specific, varying widely. The association of coronary artery disease to myxoma has not been frequently reported and may sometimes determine an early diagnosis of the tumor, before serious complications as systemic embolization or sudden death occur. Bi-dimensional and transesophageal cardiac echography are the most important tools in detecting the myxomas, while coronary angiography is mandatory for patients with risk factors for ischemic heart disease, whose clinical data raise that suspicion. Once the diagnosis has been made, the need for the operation is urgent. We present the case of a 56 year old female patient operated in our service for left atrial myxoma and coronary artery disease. The operation consisted in complete tumor removal and grafting the right coronary artery with a venous conduit. The evolution was simple, uneventful. The combined procedure may be safely performed with no increase in postoperative mortality or morbidity.

Large eustachian valve - a puzzling echocardiographic diagnosis and a difficult therapeutic management

Abstract

Anomalies of the right atrium, due to incomplete resorption of the embryologic structures, are difficult to diagnose and to establish the future management. One of these structures, the Eustachian valve, is often reported with different unexpected complications. This report presents the case of a patient with a prominent right atrial mass that was suspected at the transesophageal echocardiography to be a thrombus, but the possibility of an embryological remnant was not excluded. The patient had also significant degenerative aortic regurgitation, for which she was operated. The intraoperative right atrial inspection revealed a large Eustachian valve, which was removed. Subsequent pathologic examination confirmed the diagnostic. Although Eustachian valve is usually considered a rare condition, it should not be omitted when evaluating a patient with a right atrial mass.

Abstract

Background: Quality of life (QOL) measurements have only seldom been used in Romanian medicine. Quality of life can be assessed using questionnaires, generic or disease-specific. Material and methods: The King's College VascQOL questionnaire is specific for the peripheral arterial disease and has been used in 120 patients, before and after surgical revascularization.

Diabetul zaharat este o afectiune metabolica, a carei prevalenta este in crestere neta in ultimele decenii. Prezenta acestei afectiuni conduce la un risc cardiovascular ridicat si la cresterea morbiditatii si mortalitatii cardiovasculare. Studiul de fata si-a propus aprofundarea unor elemente etiopatogenice ale interrelatiei diabet-boli cardiovasculare si analizarea terapiei antidiabetice, a factorilor de risc si a complicatiilor cardiovasculare ale diabetului, in scopul ameliorarii evolutiei pacientilor diabetici.

Epidemiologia insuficientei cardiace

De multi ani, metodele epidemiologice au fost folosite pentru a evidentia cele mai frecvente boli cardiovasculare si a studia evolutia lor. Noile tehnici, invazive sau neinvazive, folosite in cardiologie, au permis un diagnostic etiologic mai sigur si mai precoce, dar cele mai bune criterii de diagnostic raman elemental clinice. Cu toate progresele inregistrate in cardiologie, epidemiologia insuficientei cardiace este putin cunoscuta si gravitatea bolii persista cu o prevalenta si incidenta care au tendinta sa creasca tot mai mult.

Insuficienta cardiaca diastolica

Insuficienta cardiaca este incapacitatea inimii de a asigura in conditii normale debitul sanguin nec esar nevoilor metabolice si functionale ale diferitelor organe. Inadecvarea intre functia inimii si nevoile organismului duce la cresterea presiunilor in amonte de inima insuficienta si la reducerea d ebitului destinat circulatiilor periferice, ceea ce este responsabil de aparitia semnelor de "insufi cienta cardiaca in amonte" si a celor de "insuficienta circulatorie in aval".

Blocantele beta-adrenergice au fost concepute de Sir James Black, pornind de la conceptia conform careia un medicament care ar inhiba actiunea catecolaminelor asupra cordului ar trebui sa fie eficient in terapia anginei pectorale, controlul aritmiiilor si consecintelor hemodinamice ale feocromcitomului. Pe parcursul a peste 35 de ani de utilizare, cercetarea medicala a acumulat un numar impresionant de date, ducand progresiv la cunoasterea detaliata a particularitatilor, eficientei si limitelor, care marcheaza utilizarea acestei clase farmaceutice.

Disfunctia endoteliala

Disfunctia endoteliala poate fi definita ca schimbarea fenotipului esential al endoteliului normal care asigura: relaxarea celulelor musculare netede vasculare, inhibarea agregarii plachetare, limitarea adeziunii leucocitare, inhibarea proliferarii muschiului neted vascular. In contrast, endoteliul disfunctional nu poate asigura relaxarea muschilor netezi, nu inhiba agregarea plachetara, este avid pentru leucocite, nu poate inhiba proliferarea celulelor musculare netede.
